Full name of deceased, Date of birth, Date of death, Age of deceased and Place of death
This information is used to identify the deceased and leave a post-mortem record, for example for genealogical research.
- Full name of deceased; name as you wish it to appear in the notice.
- Date of birth; use the calendar to select the date.
- Date of death; use the calendar to select the date.
Entering these fields automatically creates a sentence.
In Montreal, on June 15, 2023, at the age of 92 years and 12 days, died John Smith.

Additional description
You can write a text containing the following elements:
- Name of parents, spouse, child(ren) (spouse), grandchild(ren)
- Family
- Friends
- Colleagues
- Other important people or elements in your life
Ex: Husband of Louise-Marie Provis, he is survived by his children: Myriam (Claude Mattielli), Marc (Sylvie Moreau), Philippe (Ada Luz Suarez) and Éric (Carine Chaput). Étienne was a cheerful person who…
Let your heart speak! You can add a description, a memory, an anecdote or any other relevant information that has not been requested elsewhere in the form.

Picture
Here are the criteria to consider when choosing a photo.
- Prioritize a high-quality, clear, sharp and well-framed image. Avoid cell phone screenshots or photos taken in a frame that may cause reflections.
- Ideally, choose a photo in which the person is alone, or their face is far away from that of other people present.
- Make sure the photo is in JPG, JPEG, PNG or WEBP format, with a maximum size of 25MB.
After import, you can crop the photo as you wish.

Events related to the death
For each of the following sections, enter the events that apply to your situation,
- Location; ex: Cathédrale Marie-Reine-du-Monde
- Address; ex: 1085 Rue de la Cathédrale, Montréal, QC H3B 2V3
- Choose the date; Click on CHOOSE DATE, use the calendar to select the date. Enter start and end times if necessary.
Events
- Exhibition
This is the time when the family of the deceased receives visitors and friends to offer condolences and comfort.
If there is no viewing, skip to the next section.
- Ceremony
This is when the celebrant, or religious representative, pays tribute to the deceased. If the ceremony takes place at the same location as the exhibition, you must still enter the name of the place and the address.
If there is no ceremony, proceed to the next section.
- Burial
This is when the deceased is placed in the ground or columbarium.
If there is no burial, proceed to the next section.
- Reception
This is the time when the family welcomes family and friends following the ceremony and/or burial, to reminisce about the sweet moments spent with the deceased.
Make sure you have the exact address of the reception venue.
If there is no reception, skip to the next section.

Thanks
You can thank the people who accompanied the deceased in his life or in his last moments. (e.g. care team, nursing staff, etc.).
You can also indicate the deceased’s last wishes (e.g., not to send flowers) or a message left by the deceased.

Suggested donations
You can choose from our list, by clicking on CHOOSE AN ORGANIZATION, the organization to which you would like people to send their donations.
If the desired organization is not on the list, enter it in the second box. A member of our team will be sure to add it.
If there are no donations, proceed to the next section.

Proof of death
In order to ensure compliance of our publications, you must provide the name of the funeral service company or proof of death. Ex: Death certificates, Death certificate.
Publideces.com reserves the right to contact the funeral service company in charge of the death for validation purposes.
